As we get ready for another exciting year of athletics here at J.C. Booth Middle school please make sure that all students who plan to play a school sport have the following forms completed and turned into the correct person.

GHSA Physical Form - Turned into the school nurse
Parent Consent Form - Turned into the head coach of that sport
Code of Conduct Contract - Please take time to personally read over

Students are not allowed to participate until all forms are turned in
